Last month we reported that the CrossFit Athlete Council (CAC) supported the introduction of a medical screening requirement to compete at the 2026 CrossFit Games. The 2026 CrossFit Games rulebook, published last week, confirmed this.
All individuals and teams competing at the 2026 CrossFit Games will be required to submit a Medical Eligibility Form and Medical Consent Form prior to competing. The Medical Eligibility Form must be filled out by a U.S.-licensed physician or a Doctor of Medicine (MD/DO) for those outside the United States.

The rulebook did not include a link to the actual forms that needed to be completed. However, we have learned that CrossFit has distributed the forms to athletes, likely athletes who competed at the 2025 Games.
So, of course, we got our hands on them.Â
The Medical Eligibility Form requires the physician to list an athlete’s allergies, medications, medical conditions (e.g. asthma, Type 1 diabetes mellitus, heart conditions, sickle cell trait) and “other relevant information”.

The form also includes a checkbox where the physician can select one of three options:
- Medical eligible for the 2026 CrossFit Games without restriction.
- Not medically eligible pending further evaluation.
- Not medically eligible for the 2026 CrossFit Games.

The Medical Consent Form is signed by the athlete, authorizing CrossFit to collect information on the Medical Eligibility Form and that it would be reviewed by CrossFit’s medical staff.
The Medical Consent Form also informs the athlete that the Games Medical Director, CrossFit, LLC may, at its sole discretion, prohibit an athlete from competing to ensure the athlete’s safety or the safety of others.

Athletes who believe they will qualify for the 2026 CrossFit Games (individuals and teams only) can submit the forms beginning on February 1, 2026. All forms must be submitted by June 12, 2026. However, those who qualify via the Online Semifinal or receive a backfill invite will have until July 1, 2026, to submit their documentation.
